Scleroderma

The term scleroderma refers to a group of diseases that involve the hardening and tightening of skin and connective tissues. There are different types, some affecting only areas of the skin and others damaging blood vessels, internal organs, digestive tract, muscles, and joints. Scleroderma is usually classified as an autoimmune rheumatic disease, and is neither contagious nor cancerous. It affects four times more women than men, most commonly between the ages of 30 and 50. While there is no definitive cure, medical treatment can alleviate symptoms and improve a patient’s quality of life.

Scleroderma comes from the Greek words “sclero” meaning hard, and “derma” meaning skin, and is characterized by inflammation and stiffening of the skin. This is due to excessive collagen production. Collagen, a protein normally present in the skin, provides needed structural support in proper amounts.

Localized scleroderma is relatively mild, and it usually affects only the skin, not the internal organs. Systemic scleroderma (also known as systemic sclerosis), can also cause the connective tissue in other parts of the body—including the esophagus, gastrointestinal tract, lungs, kidneys, heart, blood vessels, muscles, and joints—to become fibrous and inflexible, preventing them from functioning properly. It is estimated that about 300,000 Americans have scleroderma, with one-third exhibiting the systemic form.

The two forms of systemic scleroderma are diffuse scleroderma, which can occur quickly and involve large areas of skin, and limited scleroderma, which develops slower and affects smaller patches of skin. Limited scleroderma is sometimes called CREST syndrome, an acronym for its common features: calcinosis (insoluble calcium deposits in the skin and subcutaneous tissues); Raynaud phenomenon (decreased blood flow in the fingers and toes); esophageal dysfunction, sclerodactyly ( hardening of the skin of the hand causes the fingers to curl inward); and telangiectasia (spider veins).

Although it is not known exactly what triggers abnormal collagen production, the body’s immune system appears to play a role; in 15 to 20 percent of diagnosed cases, scleroderma patients also exhibit symptoms of another autoimmune disease, such as rheumatoid arthritis, lupus, or Sjögren’s syndrome.

There is no single diagnostic test for scleroderma, and it can be difficult to determine which type of the disease is responsible for a patient’s symptoms. A rheumatologist will often be consulted and an antibody nuclear (ANA) test may be ordered, but because the body develops antibodies for various reasons, that test alone cannot diagnosis scleroderma. Additional tests may determine if the lungs, heart, and digestive system are functioning well and whether bones or soft tissues are being affected.

There is no known medication that can halt the overproduction of collagen—the root of the condition. Treatment instead focuses on controlling symptoms and preventing complications, including steroid creams or pills to reduce swelling and loosen tightened skin, and medications to treat gastrointestinal distress. Drugs that dilate blood vessels may help prevent lung and kidney issues, and immune-suppressants are sometimes included in a treatment regimen. Studies are now being conducted on innovative, small molecule therapies which some experts expect to make a significant mark in treatment of scleroderma by the end of the decade.
